Paging Dr. Hive... (posting this from the children's hospital)

If her symptoms worsen in the night, my advice is to take her to the er again even though you have an appointment in the morning. Something is wrong here, and I think she needs a CT to rule out abdominal obstruction, diverticulitus, appendicitis that was missed by the abdominal ultrasound, or several other things.

I agree with trulycrabby.  If naught else, she must be getting dehydrated by now.  Poor kid :-(

I have my now adult son with autism on daily Miralax (with Gatorade! Great minds think alike!) as he is largely non-verbal at home and can easily get clogged up - when constipated he does act miserable, vomits, loses appetite, etc.  I have been to the ER with him and had him scanned and been told he is totally clogged up. That is when I add Milk of Magnesia (in pill form as he refuses the liquid and I don't blame him) and bits of an ex-lax bar.  Eventually the "plumbing" gets unclogged.  But your dd is vomiting far more than my ds ever did, so I think even if constipation is part of the problem, it is not the only problem. It is just a side problem.

I'm just so sorry for you both. I know how worried you must be. I would be prepared to have a hissy fit, get her admitted for IV's and a GI doc ASAP. Prayers for you both.

Yes, thinking this totally warrants a hissy fit if necessary! Poor kid has got to be dehydrated and miserable. If you can get her admitted, they tests can be done more quickly. Good luck, and hope you guys get some answers/results soon:(

After this morning's visit, we are considering a Zoloft side effect. We added Zofran and will taper off Zoloft. If that doesn't stop the vomiting, then we know it's something else.

Ruled out:

Appendicitis

Bowel obstruction

Virus

Dd has not lost weight, which surprises me given how little she's been able to keep down.

Shoot! I'm so sorry to see this. I hope you get called in off the cancellation list for your GI soon. In the meantime, I would keep track of what she is eating and her activities: times, etc. and how it relates to her vomiting. Hopefully you can start seeing some patterns that will help you figure things out.

Thanks for the update! So glad for the improvement. Before my daughter was diagnosed with FPIES, they ruled out Cyclic Vomitting Syndrome. You might want to check it out. https://www.niddk.nih.gov/health-information/health-topics/digestive-diseases/cyclic-vomiting-syndrome/Pages/facts.aspx#what

My friend's son was diagnosed with this a few years ago. He suffered for over a year then a nurse asked if he'd ever been tested for diabetes. Turned out he was actually diabetic and was going through vomiting cycles related to blood sugar drops.

We already have an appointment for tomorrow. I want to at least do some blood tests to see if there is anything to explain what's happening. It was only a guess that she was reacting to the Zoloft. She had a quarter dose Friday morning, but I haven't given her any meds of any kind since then. I understand that Zoloft will take at least a week to completely clear, but a pharmacist friend of mine said most of it would clear within the first three days.

We wrote down approximate times and foods she ate today and I asked her to keep a record going forward. She can't function like this. She's missed two full weeks of school. She can't go to social events. It's terrible. :(

Regarding possible chronic constipation and Miralax, my eldest was/is prone to this. He had encopresis from it as well.  When he got backed up, we would do basically a small bottle of Miralax over the course of a day....mixed with Gatorade.  Sometimes even Fleet enemas as well.  It would take that much to get him going.  The "normal" dose 1x or 2x/day was just a maintenance dose.

Here are some other regimes from various pediatric specialists:

Seattle Children's is similar but they also add Dulcolax.

My poor girl is so tired of being sick. She's had visual disturbance (associated with headache), which is why we started trying a migraine med today. Maxalt made her headache go away, but she still threw up a couple of hours later. A couple of hours after that, she threw up again (no food in between times). The headache returned, so I just gave her another dose of Maxalt. Hopefully she will get some sleep. No doubt, she will wake at least once to be sick. 😥

The longer this goes on, the more worried I get about what's wrong. As we rule things out, the scarier or more life-altering explanations are the ones that remain.

:grouphug:   no suggestions, but I sympathize.  1ds had something similar that went  on and off for many months before we got a diagnosis. including trips to the ER, IM narcotics and IVs for hydration.  he had a pinched nerve from a fractured vertebra.  (the drs ignored the fracture because it was healed.).

I'm sorry you're all going through this. Just throwing it out there to see if they checked for H. Pylori infection. It'd be pretty unusual to have so much vomiting but you never know. If you're seeing a pediatric GI then at least you're in the right place.

Has she been checked for serotonin syndrome?  This a are major, all be it rare, side effect when you combine or even take some of the medication you are talking about.  Zoloft on it's own.  Zoloft+Maxalt, Zofran+Maxalt.  I have no knowledge of her other meds, but others can contribute, even vitamins and supplements.

Blurred vision, headaches, nausea, vomiting, diarrhea or constipation, etc. can all be signs.  The timeline you mention could work, and the meds you are using to relive symptoms could continue the cycle.

We've discussed serotonin syndrome. She hasn't had Zoloft or Zofran since Friday morning. She did have Maxalt twice last night, but this list symptoms from the Mayo Clinic doesn't fit:

http://www.mayoclinic.org/diseases-conditions/serotonin-syndrome/basics/symptoms/con-20028946

She threw up again an hour and a half ago. At this point it seems predictably worst between 4pm - 7am (3-5 episodes most evenings/nights).
