We homeschooled year-round so it was uneventful really and the kids never knew what grade they were in for sure.

How about the basic card game "War" but turn over two cards each play. Highest wins. or Lowest wins
Works for Addition, Subtraction, multiplication, division, fractions too.

New habits take time to develop.
It's been almost 3 years and she hasn't stayed on task so this is nothing new.
It can be helpful to look for small improvements over time.
