A lot of the computers are running complex internally written software that an upgrade would render useless. The software has to be completely rewritten to work with the new operating system. I know right? Keep up with your updates people! With the hospitals, though, the issue was that their proprietary software has to be tested with an update before it is deployed and that means it can be quite a delay. They usually do, but that doesn't mean they can access it in the middle of the terminals being locked down, as was the issue yesterday, and they cannot protect the sensitive data that is copied even if they have second working backups.
