Because of the drugs or because something about her was subconsciously throwing him off? Something like she was his sister?! And I caught that the therapist looked like the woman Sherlock met, but not the same as the bus woman. I thought when she took her wig off that she was going to reveal herself as a man... Brother maybe? I do remember her saying she and Smith met through a mutual "friend" of sherlocks. Presumably Moriearty? I need to rewatching too. This would be so like them, to dangle a fourth but make us wait.
