I've had this happen twice recently at different practices. I was not comfortable in agreeing to this
, so while I signed the paperwork for other items appearing on the page, I circled the part giving blanket consent, initialed it and wrote that I would give consent for procedures, treatments, medications, etc., on a case-by-case basis, after discussion with the doctor. In both instances, the appointments were for my children. One practice looked over the form and then filed it. The second practice questioned my concern and acted as if it was strange that I had a problem with it. I have always carefully read any form that I've signed and thought it was odd that they were now asking for this type of authorization.
