It's called "The Spelling-Book Consisting of Word in Columns and Sentences for Oral and Written Exercises Together with Prefixes, Affixes, and Important Roots from the Greek and Latin Languages" by William D. His "valuable reading programs from the past" page includes a phonetics text and " Hyde's Derivation of Words: with Exercises on Prefixes, Suffixes, and Stems" http://www.donpotter.net/education_pages/valuable_reading_programs_f.html I also like his Latin links.
