If I'm talking to someone (either in person, or letter), I'd use the general term for their rank. Ex: General Smith, Colonel Smith. Formal addresses, required for any time a name is written to be seen publicly or to be said in a ceremony, would use the full title: Brigadier General Smith, Lt.
