Running Injury (what could it be)?

My DH has been training for an upcoming half marathon, not his first. He's been running since December. 

 

A bit over a week ago he ran 6 miles after having a running break for a week. The day after that run he had pain and swelling on the outside of the shin bone above the ankle. He has had issues there in the past, so thought he could run through it as he has before.

 

It wasn't getting better, so he stopped running for several days. The pain was improved, though not the swelling.

 

He ran Saturday, and the pain came back full force during the run and remained. The pain is substantially better in the last 24-36 hours, but the swelling has actually increased.

 

He's now swollen on both sides of the shin bone from ankle to mid calf. It's pitting (the indent you get when you press remains). Pain is better every day, though it's still slightly painful and definitely touch tender. He feels tightness, probably due the the swelling. There has never been any bruising.

 

What could it be? Would urgent care be warranted? Could they handle something like this? Should he try to get into ortho instead? Wait it out?

If you don't mind my thinking out loud, I'd first think plain ol' shin splints - that would seem the most likely.  But, I'm not particularly familiar with visible swelling accompanying it - that would point to seeing an ortho if it doesn't improve (stress fracture? odd that the swelling is increasing on a break from running)

 

The second thing that comes to mind would be a clot in the foot/ankle/calf but I think that's usually in the back, not the front of the shin(?) and that would require an ER visit.

 

The third thing that comes to mind would seem the least likely, and that would be something of an infectious nature of the type from various insects (I don't want to go there and in any case, I'm not sure that swelling is the issue in such cases as much as shin bone pain).
