You could print one you like, take to staples and have copies made (just on one side so the back is blank) and have them either spiral or comb bind it for you. I know it sounds like a pain, but sometimes it's easier to make-it-yourself than spend a bunch of time looking for just the right thing. The site listed below has single lined paper and also the double lines with the dashed line in between. You can adjust the spacing and other options such as line color.
