Courtney Whitmore, a Masters degree candidate at the University of South Carolina School of Medicine is conducting a research study entitled Evaluating the Social Informational Needs of Emerging Adults with Genetic Conditions. The study intends to allow providers to better understand the patient perspective and the informational needs of these individuals in regard to the decisions they now independently face during their transition to adulthood.

Participants must be 18-26 years of age and have achondroplasia, cystic fibrosis, sickle cell anemia, or Marfan syndrome. The questionnaire is completely anonymous and upon completion participants have the option to be entered into a raffle for a $50 Amazon gift card. Please complete the questionnaire at: https://www.surveymonkey.com/r/socialtransition. Thank you.
