Question about altering a dress

I have a once-in-a lifetime function to go to: A ball!

 

So, I have purchased a long dress, and it is gorgeous, but needs some alterations. It is a long-sleeved, crepe de chine bodice, cinched waist, with a plisse/pleated satin skirt. Fits great, but seems a bit large under the arms. Can a good tailor fix this, or do I need to just get a smaller size?

 

Here's the dress: It's my Christmas, birthday, anniversary, and mother's day gift for the next year, and I snapped it up on a one-day sale, so I didn't pay the shocking price that's listed on this site:

Oh, it's also going to be my mother-of-the-groom dress, so I will get at least two wears out of it.

All my dresses need to be altered or tailor made. If it is only a bit large under the arms, a tailor would be able to alter the dress to fit better. My aunt would just temporarily hem it so that when hand me down the next person can let the thread out and wear.

 

If it is loose at the waist and bust line, then a smaller size is likely to fit better as those alterations are harder to get done.
