It's free and done on the computer. - The Phonics Page offers several free printable tests that you can administer yourself. - The National Right to Read Foundation has a free Reading Competency Test you can print and administer. - If you're willing to spend a little money to have a more detailed assessment, you can try the DORA (Diagnostic Online Reading Assessment).
