We have an HSA, and I know that there is a modest annual fee for the debit account part of it. Maybe 20/yr in my case, but I shopped around and chose the lowest fee provider I could find. (Health Savings Administrators). (You might not have had a choice, though. I am the administrator of my accounts, as we own the business.)
