I have to submit an annual report to the school district along with work samples but can't seem to find an editable template anywhere. I removed our school logo and personal info. I hope it's helpful. ETA: The PDF File is a bit bulky and someone experienced difficulty viewing it so, if you're interested, just send me a PM and I'll get it to you that way. Disclaimer: Homeschooling is situational for us and our children are likely to return to a brick and mortar schools for junior high or high school. I want their records to be consistent with their earlier school report cards and thorough enough to facilitate placement. This level of detail is not required by law and it may be too much for some.
