WTMindy
I'm looking for a nice template for the kids to record their experiments. What are your favorites?
Nestof3
Did you want something more involved than this? edit: the forms are not accessible through the website I posted -- the link expires or something. Just go to www.about.com enter "experiment form" in the search engine and you will be able to access the two pages. If you would prefer to PM me with your email address, I can email you the two .pdf pages.

Kate in Arabia
I have one that is slightly more "graphic-y" that I designed when putting together a chemistry lab book. I tried to attach it, but the file size is just a bit too big; if interested I'd be happy to send it to you...

Depending on what you are looking for there are various resources along this line on the Donna Young site: http://www.donnayoung.org :)
