Momof3Maidens
I'm needing one to include with my daughter's high school records. I created one but it doesn't look very official. Thanks!
Teachin'Mine
I would make a simple one using Excel. I'm guessing the one I had linked wasn't what you're looking for. :) ETA: Here's another site with templates. http://www.edutech.nodak.edu/powerschool/templates/report_cards/
Momof3Maidens
I did take a look at those. I like the one that lists all the grades from K-12 although all I'm wanting is grades 9-12. Maybe I should venture out and try to just duplicate that on my own. I'm not very computer savy when it comes to creating documents and such. I'll give it a try and see what I come up with. Thanks so much for your advice:)
Momof3Maidens
Oh! Forgot to check out the other link you gave. I think my mind is fried. :)
meet me in paris
I just used this one from Donna Young and turned it in to my county: http://donnayoung.org/forms/planners/grade.htm It's on the bottom of the page. If you use the Word/.doc version vs. the PDF you can edit all the fields (I made a lot of changes, but I kept the format). The Superintendent's office just emailed me this morning and complemented my records. ;)

Momof3Maidens
 Ok. I composed one using a couple of templates that you ladies suggested and it looks good so I'm pleased with it. Thanks so much for your time! Teachin'Mine - You weren't on ignore, thanks again!
