Homeschool Planner - Excel Spreadsheet Template Needed Please

Do any of your have an excel template that I could use to help us plan? I've searched all over the internet and cannot find something that I'm looking for.

 

I would love to see what all you have used and how I can download a copy of it.

I must say that scholaric does everything I wish I knew how to make an excel spreadsheet do. It costs $5 a month for up to 3 kids, but you get a free months trial. Totally worth it to me.
