I was not asked to provide it when I filed my taxes either. So I'm assuming I don't need to do anything more than throw it in my file. If you e-file (and who doesn't?), there will be a spot to check off the number of months you were each covered by health insurance. LOL It was there somewhere. It might have been a single checkbox like "did (Wendy, mr. wendy, Son 1, son 2) have health insurance all 12 months?" If you checked yes, it fills the schedule for you. If you checked no, it would drill down and ask who was covered which months. It probably also asked you if you were covered under medicaid expansion or if you bought a plan in the exchange for your state. I'm pretty sure your state has an exchange. And because you are not you clicked no, along with all of the other forgettable checkboxes about being a beneficiary of a foreign trust and if you have any gambling winnings to report, lol.
