We had noticed she was having trouble and you suggested more Phonics, I did a quick assessment, and realized she was totally forgetting her Phonics. After considerable thought and research I settled on Rod and Staff's "Developing Good Reading." it is a 6 - week intensive remedial phonics course that starts with ear training and uses workbook, ear exercises, flash cards, and review. So far we've already discovered major blind spots, and the TM is very thorough! For instance she needed more training on hearing short /e/, and on short and long vowels in general. It's a very thorough, easy to use, and intense crash course on phonics. Very well done! My dd is having trouble with syllables and always has had trouble telling the difference between short e and short i, but I think that's because we live in the South!
