I linked to one page to get you going, but spend some time looking at the different forms there. http://donnayoung.org/forms/planners/index.htm I have been making my own (obsessively ;)) since I figured out how to do it. I don't use a spreadsheet. I generally make them in Microsoft word, using a table format. I am not particularly computer savvy either. I could probably send you one if you thought that would help or talk you through it. However, I still would suggest starting with Donna Young's website, because that's an easy way to go.
