PSAT notarized form?

I just stumbled on this document from the College Board website.  Do we need to bring it to the PSAT tomorrow?  It says it needs to be notarized?  

 

It was linked from this website on their list of things homeschoolers need to bring on test day.  

 

I did the first time DD took the test (she now has a state ID card.)  I honestly don't think the administrator at the school would have insisted, but I wanted to be sure. If the test administrator is a stickler, you'll need either the notarized form or an official ID.
