EKS Posted June 21, 2017 Share Posted June 21, 2017 I am looking at my copy of the form I filled out in 2010 (I don't know if it has changed). Fill in Section I. Be sure that your high school code is the homeschool code. For the school name, I just said "Homeschool" because I didn't want to give them any chance to get confused about that. Ignore the stop sign at the bottom of the second page. Fill in Section II part A. I only listed diagnosed disabilities that were listed in the documentation I was submitting. Skip Section II part B. Fill in Section III part A. I asked for all accommodations suggested in my son's documentation and he received all of them (except that double time was reduced to 150% time). He ended up only using the extended time accommodation. Skip parts B and C. Submit with your child's documentation. It isn't at all obvious how homeschoolers are supposed to fill out the form (as I'm sure you noticed!). I'm pretty sure I called the College Board and found out what to do.
