But here is the format that we used in honors chemistry in college:

 

Title

Abstract--this is a summary of the results of the experiment, that also includes the rough summary of the experimental procedure itself. All of this information will be repeated later in the report. It is often best to compose this last.

Purpose: This is a more detailed rendition of what theory is tested by the experiment and how that is done.

Materials list and materials handling requirements/sensitivities

Procedure: Exactly what was done, including exact quantities of each chemical.

Observations: These are the detailed observations of the results of each action in the procedure. Language here is fairly formal and limited to the observation itself, not to a conclusion; i.e. "No change noted" is favored over, "Nothing happened."

Conclusions: What the experiment proved and how it proved it. This must be very specific and strong, and completely avoid assuming facts not in evidence.
